Empirical 10-day sequential H. pylori eradication therapy could allow sparing of surgical intervention in chronic rhinosinusitis patients

Objective: To determine the association between H. pylori (Hp) infection and subjective and objective chronic rhinosinusitis (CRS) parameters and to evaluate the effect of empirical Hp-eradication therapy using the 10-day sequential therapy (ST) on these parameters.<br />Patients and Methods: The study included 85 patients with manifestations of CRS, completed questionnaire to determine total subjective score (TSS) and underwent flexible nasal endoscopy, CT imaging, Urea Breath Test (UBT) and stool Hp-antigen test. Patients were divided as Hp+ or Hp-. All patients received 10-day ST and responders were re-evaluated 3-m later, but non-responders were given another 10-day ST course and re-evaluated. Non-responders to ST courses and responders who developed recurrent symptoms underwent FESS.<br />Results: 17 patients (20%) were Hp+; 11 in study and 6 in control groups. 12 Hp+ patients (70.5%) and 23 Hp- patients (33.8%) responded to ST, 5 Hp+ patients were assigned to 2nd ST course and 48 patients underwent FESS. Three of Hp+ patients responded to the 2nd ST course, while two underwent FESS. Seven of the responders to 1st ST course had recurred manifestations and underwent FESS, while 27 patients completed their follow-up uneventfully.<br />Conclusion: Hp infection may underlie chronicity of CRS and is correlated with severity scores. 10-day ST for Hp eradication is being effective within short-time trial. Empirical ST provided improvement of CRS scoring and spared or postponed surgery in about 31.8% of patients, irrespective of being Hp+ or Hp-, so it is advocated as a first-line trial for patients presenting with symptoms suggestive of CRS

Background: Upper respiratory tract infections have been regarded as the most frequent illnesses affecting people worldwide. Data about bacterial profile and current antibiotics resistance status for such infections is scarce in Ethiopia.<br />Objective: This study was done to determine bacterial profile and their antimicrobial resistance patterns in patients with upper respiratory tract infections.<br />Methods: A cross sectional study design was used and was conducted in Debre Berhan, Ethiopia from October 2016 to September 2017. Patients with upper respiratory infections presented to outpatient department were enrolled for the study. Socio-demographic and clinical data were collected using standardize questionnaires. Clinical samples of throat swabs were collected and transported to the microbiology laboratory for processing. Data was analyzed using SPSS version 20.<br />Results: A total of 109 patients, almost equal number of male (49.5%) and female (50.5%) participants were involved in the study. Majority (62.4%) of the participants were with age of below 5 years old. The culture positivity was 36.7%. Among all bacterial isolates (n=40) the predominant isolate was Streptococcus pyogenes (42.5%), and followed by Streptococcus pneumoniae (22.5%), Staphylococcus aureus (15%), Haemophilus influenzae (12.5%), and Klebsiella pneumoniae (7.5%). Overall antimicrobial resistant pattern seen in all bacterial isolates ranges from 0 % to 66.6%. High resistances in all isolates have been seen. Cough and watery eye were found as predictor variables.<br />Conclusion: High number of bacterial isolates and emergence of resistant strains have been found. We recommend to taken this in accounting during medical decision, guideline development and designing interventional strategies.

Introduction: Young children who experience severe-to-profound sensory-neural hearing loss (SNHL) face challenges in developing spoken language since they are unable to detect acoustic-phonetic cues which are essential for speech recognition.<br />Aim: The aim of this study was to examine the influence of age at implantation on children's language development to detect whether early intervention with cochlear implantation in children with severe to profound SNHL results in better linguistic outcomes.<br />Subjects and Methods: 60 children from the cochlear implant unit from the Kasr El Ainy hospital were recruited to participate in this cross sectional study. They suffered from congenital bilateral severe to profound sensori-neural hearing loss since birth. They all had unilateral cochlear implant. Their ages ranged from 2 - 7 years old, 30 of them received cochlear implant prior to 5 years of age and the other 30 children received cochlear implant after 5 years of age. They all underwent formal language assessments.<br />Results: Comparison between group I & group II in receptive, expressive and total language ages obtained by the Arabic Language Test with & without visual cues showed a significant difference between group I (early) and group II(late).

Occurrence of lipoma is often sporadic and rarely inherited<br />Case report: A 74 year old male presented with a left sided neck swelling which was gradually increasing in size over a duration of 10 years. It was associated with intermittent pain 6 months prior to presentation.<br />Discussion: Lipoma is the commonest benign mesenchymal tumor and is a relatively uncommon presentation in the head and neck region. Often a fine needle aspiration cytology is able to indicate the presence of a lipoma. However, with the computed tomographic scan, the level of attenuation is able to indicate the presence of a lipoma. Although benign, there are reports of malignant transformation to a liposarcoma.<br />Conclusion: Lipoma of the parotid gland is often mistaken for other tumors hence an accurate clinical assessment together with preoperative imaging and histopathological examination helps to facilitate the diagnosis

Background Thyroglossal duct cyst is the most common midline congenital swelling in the head and neck. This anomaly is very rare in elderly. It usually presents as a midline painless neck swelling. In rare cases, it may show abnormal extension into the larynx that manifests with dysphonia and laryngeal obstruction. Case report We report a case of thyroglossal cyst in a sixty five year old patient who presented with difficulty in breathing, and hoarseness of voice. Fiber optic laryngoscopy revealed fullness in the right vallecula and the right piriform fossa. The differential diagnosis on Ultrasound and Computed Tomography was thyroglossal cyst versus laryngocele. After surgical excision, dyspnea resolved and voice improved. Postoperative fiber optic laryngoscope revealed the disappearance of previous fullness. Histopathological examination confirmed the diagnosis of thyroglossal cyst. To the best of our knowledge, very few cases of thyroglossal cyst with laryngeal extension were encountered in literature, and in this study, we are reporting another case of this extremely rare condition.

Background: Pneumolabyrinth consequent to a temporal bone fracture is a rare entity. It occurs when air enters the vestibulocochlear system due to a pathological communication between the middle and the inner ears. The diagnosis is usually confirmed radiologically using high-resolution computed tomography imaging. Pneumolabyrinth is categorized radiologically as follows: vestibular pneumolabyrinth, cochlear pneumolabyrinth and combined pneumolabyrinth.<br />Case Presentation: We herein describe the clinical presentation and management of pneumolabyrinth in a 43-year-old man, following an otic capsule-violating temporal bone fracture. After initially caring for the patient conservatively, without improvement, surgical intervention was undertaken. Unfortunately, hearing was irrecoverable.<br />Conclusion: Delaying medical and surgical intervention and the location of the air bubbles associated with the combined type contribute to the irrecoverable loss of hearing. As otic capsule-violating fractures might not be observed initially on brain CT scans, high-resolution and thin-slice CT scans of the temporal bone is crucial when suspecting temporal bone fracture.

The incidence of radiation-induced sarcoma (RIS) in head and neck region is increasing in trend nowadays. There are few cases have been reported. However, the exact aetiology of radiation-induced sarcoma (RIS) is still unknown. We report two cases of radiation-induced sarcoma (RIS) which the primary malignancy were basal cell carcinoma of the cheek and nasopharyngeal carcinoma. These patients had received a total of 70Gy and 65Gy radiotherapy for 13 years and 6 years back respectively. Histopathological examinations for both of these patients revealed a spindle cell sarcoma and a high-grade sarcoma respectively. Both cases were inoperable in view of advanced disease at presentation. We will discuss on the clinical findings, histopathological findings and the main characteristic of radiation-induced sarcoma (RIS) in imaging. The dilemma of managing radiation induce sarcoma in head and neck patient will also be discussed as managing previously radiated head and neck primary is undeniable challenging

Simplification of the severity hierarchy score for pediatric sleep apnea screening

Pediatric Obstructive Sleep Apnea is a common condition. However, it remains under diagnosed exposing to associated morbidities. Polysomnography is the standard diagnostic test, but its availability is sometimes limited, especially in poor countries.<br />The questionnaire proposed by Spruyt and Gazal has been validated as a screening tool by many studies. A score above 2.75 have an 82% sensitivity, a 81% specificity and a negative predictive value of 92% for detecting moderate OSA. However, the way of calculating the score seems to us rather complicated and may limit its use in daily practice.<br />Therefore, we propose, for calculating the score, a table that may be printed in sheets of paper. We propose a simple modification that could, in our opinion; greatly facilitate the use of this screening tool by reducing the use of calculators and the risks of errors. In this way, we hope to make its use much easier in everyday practice
