Factors affecting occurrence of post intubation and post tracheostomy tracheal stenosis in intensive care units

Objective: Iatrogenic tracheal stenosis is a life threatening iatrogenic complication of prolonged endotracheal intubation and tracheostomy in intensive care units (ICUs). It is one of the toughest morbidities in otolaryngology.Aim of the Study: This prospective observational study was conducted to detect the factors affecting occurrence of tracheal stenosis in patients subjected to tracheostomy after endotracheal intubation in ICUs.Patients and Methods: Sixty eight patients were included in this prospective study. They were admitted to (ICUs) and were subjected to endotracheal intubation, mechanical ventilation and tracheostomy due to prolonged intubation during their stay in (ICUs). The patients were followed up after being discharged for six months to detect incidence and factors affecting occurrence of tracheal stenosis. They were followed up using flexible fiber optic tracheobronchoscopy under local anesthesia, just after weaning from mechanical ventilation three and six months later.Results: Of the studied patients, 50 patients had no tracheal stenosis until the end of follow up period (without tracheal stenosis (WTS) group) while, 18 patients had tracheal stenosis (TS group). The mean duration of mechanical ventilation in WTS group (12.1 ± 7.6 days) was statistically shorter than that of TS groups (16.6 ± 10.4 days) (P = 0.048). Tracheostomy stoma wound sepsis was recorded in 4 patients in TS group and in 2 patients in WTS group.Conclusion: The main predisposing factors for post intubation and post tracheostomy tracheal stenosis are tracheostomy stomal wound sepsis and prolonged duration of mechanical ventilation. In contrary, tracheal cartilage injury during tracheostomy, age, sex and reason for mechanical ventilation was found to have no role in occurrence of tracheal stenosis.

Cognitive Communicative Abilities in Tinnitus Patients with Normal Hearing

Background: Tinnitus is a bothersome complain , some studies show that 13-18 percent of the people suffer from tinnitus, almost 5 percent of which, have chronic or bothersome tinnitus. Chronic tinnitus may be accompanied by anxiety, depression, insomnia, and impairment in cognitive functions such as memory and attention.The Aim of this Study: To compare the auditory memory and auditory attention between two groups with and without tinnitus all of them have normal hearing.Methods: This study were performed on 30 tinnitus patients (10 men and 20 women) aged from 20 to 50 years old and 30 healthy volunteers (12 men and 18 women) aged from 21 to 50 years old with normal hearing thresholds. Pure tone audiometry (PTA), auditory continuous performance test (ACPT), Memory for recognition (MR), memory for content (MC) and memory for sequence (MS) were employed for all participants. Tinnitus evaluation and tinnitus handicap inventory questionnaire (THI) was done for the study group only.Results: Comparing the results of ACPT between study and control groups show no statistically significant difference. While comparing the results of memory battery test between both groups were statistically highly significant.Conclusion: Tinnitus decreased the memory battery test scores at the study group in comparison with the control group. Auditory continuous performance test (ACPT) results show no difference between study and control group. Duration of tinnitus affect significantly ACPT results. Bilateral tinnitus affects memory for sequence results. While loudness decreased results of memory for recognition.

Hearing results after ossiculoplasty with incus transposition, cortical bone autografting and bone cement

Background: This study assesses hearing results after ossiculoplasty with incus transposition (IT), cortical bone autograft (CBA) and bone cement (BC).Methods: A total of 21 patients with mild/moderate conductive hearing loss were enrolled in our study. Patients were assigned to each of the three techniques according to length of incus defect. Pre-operative and 6 months post-operative audiograms (PTA) were obtained and results of air bone gap (ABG) were statistically analyzed.Results: A total of 7 patients were included in each technique. BC ABG was decreased from 26.57 (5.06) (mean (SD)) to 10.86 (6.34) with mean gain 15.71 (5.85) For IT group, the mean for pre- ABG was 33.57 with SD 5.13, it became 15.86 (1.86) after 6 months of follow-up with 17.71 (3.40) gain. For CBA, pre- ABG mean was 35.86 (5.67) that finally became 19.14 (2.41) achieving ABG gain 16.71 (6.68). No significant statistical difference between pre-post gain in all three surgical procedures (p-value = 0.80). Regarding side effects in each technique, infections was noted in one case of IT and two cases of CBA, transient infection was found in one case of IT and CBA, and transient vertigo in one case of BC with no significant difference in adverse effects between the three group (P-value= 0.494).Conclusions: Both BC, IT and CBA are reliable techniques for ossiculoplasty with good postoperative hearing results. The choice of the proper technique depends on length of incus defects

Prevalence of Otitis Media Among Patients Attending Otorhinolaryngology Clinic in Ramadi City/Iraq

Objective: This study aimed to evaluate the prevalence of various types of otitis media.Patients and Methods: A cross-sectional observational prospective study conducted in AL-Ramadi city/Iraq during the year 2018. Individuals who attending an Otorhinolaryngology outpatient clinic with the diagnosis of acute suppurative otitis media (ASOM), otitis media with effusion (OME) and chronic suppurative otitis media (CSOM) were analyzed. While any subject or their relatives who don’t wish to participate in the study was excluded.Results: The age of the patients ranged from 1 month to 76 years (mean age=10.2 years±9.027). Five hundred forty-seven (54.91%) were males and 449 (45.09%) females (male to female ratio, 1.2:1). Out of 7292 cases attending Otorhinolaryngology outpatient clinic, a 996 (13.65%) cases had otitis media. During February, the peak frequency of OM was seen 122 (23.64%), while during April the least frequency 59 (9.57%). OM was seen in around 50% in the age group > 18 years. While, only 225 (22.59%) in age group < 5years. Four hundred and four (40.56%) had OME, 331 (33.23%) had CSOM, while 261 (26.21%) had ASOM. The observed distribution of the OM was left OM (37.56%, n=374), right OM (34.63%, n=345) and bilateral OM (27.81%, n=277).Conclusions: Otitis media has a prevalence of 13.65% among patients attending the Otorhinolaryngology outpatient clinic. Lack of guidelines in Iraq for treating OM has a drawback in the management of this common disease.

Open-Nasality and Eustachian tube Dysfunction After Acute Weight Loss in Bariatric Surgery

Velopharyngeal Insufficiency (VPI) means that the velopharyngeal closure is inadequate or disturbed. VPI may be organic or functional and results in open nasality. VPI due to sudden weight loss is a very rare condition. We reported a case of open nasality and Eustachian tube dysfunction due to sudden weight loss after bariatric surgery.

Hemorrhagic Auricular Schwannoma: A case report

Introduction: Schwannoma is a benign tumor of Schwann cells and is seldom to be seen in the auricle. In addition to the rarity of auricular schwannoma (AS), intratumoral bleeding is never previously reported.Case report: A 35-year-old female patient who has presented with right painless auricular mass for 2 years. A sudden and rapid increase in its size occurred in the last 2 months before presentation. The mass was excised under general anesthesia. Histopathological examination proved the diagnosis of AS.Discussion: Schwannoma is a slow-growing benign tumor. It commonly affects the head and neck region between 25-45%. And vestibular schwannoma is the commonest type of schwannomas in this area. The first case of AS was reported by Fodor et al. in 1977. Since that time only 4 cases were reported in the world. Bleeding within schwannomas e.g. vestibular schwannoma was reported, but it is not reported in case of AS. Therefore, this hemorrhagic AS was the first reported case in the world.Conclusion: AS is seldom seen in clinical practice. And intratumoral bleeding is never reported before.

Primary Thyroid Lymphoma Presenting in Stridor

Primary thyroid lymphoma is an uncommon disorder of the thyroid gland. It is an aggressive yet potentially curable malignancy when diagnosed early and treated appropriately. We present three cases who presented with rapidly enlarging neck swelling of approximately one month duration and stridor. Contrast enhanced computed tomography of neck showed enlarged thyroid gland compressing trachea and displacing major vessels. All three patients underwent isthmusectomy and tracheostomy under general anaesthesia. Histopathology showed features of diffuse large B cell lymphoma. They were planned to receive chemo-radiation. Two became seriously ill and one was lost to follow-up. Therefore, early consideration should be given to the possibility of primary thyroid lymphoma in patients presenting with rapidly enlarging neck swelling and stridor. Contrast enhanced computed tomography provides quick useful information to plan for surgical management of airway and immunohistochemistry of tissue provides key information in making the diagnosis. Prognosis is grave when the patient presents late.

A Rare Cause of Retropharyngeal Abscess: Cervical Pott’s Disease

Tuberculosis (TB) disease is commonly caused by Mycobacterium tuberculosis.Tuberculosis is known to affect almost every organ in the body, but its manifestations in the head and the neck region are quite rare. Tuberculous retropharyngeal abscesses are exceptional. They are usually secondary to tuberculosis of the spine.We report one case of cervical Pott’s disease revealed by a retropharyngeal abscess. The clinical presentation was dysphagia with bilateral neck swelling. Radiological findings suggested the diagnosis, showing a retropharyngeal collection with vertebral osteolysis. The histological assessment confirmed the diagnosis showing several granulomas with central caseous necrosis. The evolution was favorable after drainage and anti-tuberculosis drugs.Vertebral tuberculosis is rare. Cervical Pott’s disease is exceptional, and retropharyngeal abscesses can be the revealing feature of this condition. Symptoms are not specific. The diagnosis is based on radiological and bacteriological assessment. Treatment implicates a multidisciplinary team, first the evacuation of the collection, then anti-tuberculosis chemotherapy
