Anxiety Profile in Children and Adolescents with Stuttering

Objectives: To compare anxiety levels in children and adolescents who stutter with non-stuttering subjects.Methods: The study included 35 stuttering patients and 35 age and gender-matched control group. Both groups underwent subjective assessment of anxiety using Taylor anxiety scale. Stuttering severity was assessed in the patients’ group using the Arabic Stuttering Severity Instrument (A-SSI).Results: Stuttering group showed significantly higher anxiety levels than control subjects (p < 0.0001). Taylor Anxiety scale results were significantly higher (P<0.05) in female stuttering patients than male patients. Anxiety scores showed a statistically significant negative correlation with age of onset of stuttering (r=-0.48, p = 0.002) and a statistically significant positive correlation with duration of stuttering (r= 0.329, p= 0.046).Conclusion: Children and adolescents with stuttering have significantly higher anxiety levels compared to control subjects. High anxiety scores were significantly related to early onset and longer duration of stuttering.

Management of Children Referred to ENT with Suspected Obstructive Sleep Apnea Following Overnight Oximetry.

Background: Obstructive sleep apnea (OSA) has been associated with increased risk of respiratory complications after adenotonsillectomy in children. Not all such children require overnight admission, and same-day surgery is appropriate for some patients. Selection of those that are suitable for same day discharge relies on accurate identification of OSA severity.Guidelines about ‘Day Care Adenotonsillectomy in presence of Sleep Apnea’’ provided by Nottingham Children’s Hospital, the UK, published in 2014, and applies to those children who had a sleep study performed pre-operatively.A typical cost-effective practical sleep study entails at-home overnight pulse oximetry.Aim: The aim of audit study was to see if suspected OSA is appropriately managed at Trafford General Hospital (TGH), the UK, according to the standards provided by Nottingham Children’s Hospital, in addition to taking one standard from the Royal College of Pediatrics and Child Health report, on Standards for Services for Children with Disorders of Sleep Physiology, published in 2009.Study Design: This is a retrospective local audit study focused on children who were referred to ENT at TGH for suspected OSA or who were suspected of OSA during ENT follow-up for another non-OSA reason.Conclusion: There is good practice in excluding insufficiently analyzed oximetry.Non-compliance was mostly due to mild OSA being ruled in on an inconclusive study. The listing and referral for adenotonsillectomy is appropriate despite conflicting local practice to refer all children with suspected OSA to a specialized centre.

Post-exposure effect of music through personal listening device (PLD) on speech perception in noise test in young adults

Introduction: Tinnitus is often perceived temporarily after noise exposure, usually disappearing within a few hours. However, a significant number of young people perceive permanent tinnitus which is the most frequent symptom following exposure to noise via PLDs.Objective: To evaluate the effect of music exposure through (PLD) with and without tinnitus on results of EHFA, TEOAEs, ABR and SPIN tests in normal hearing young adults.Patients and Methods: Sixty normal hearing young adults were included in this study, fifteen as a control group and forty-five PLDs users as study group, the study group were subdivided according to the presence of tinnitus into two subgroups. All subjects were submitted to basic audiological evaluation (pure tone audiometry, speech audiometry and immittancemetry), Extended high frequency audiometry (EHFA), transient evoked otoacoustic emissions (TEOAEs), auditory brainstem response (ABR) and speech perception in noise (SPIN) test.Results: There were statistically significant differences in extended high frequency audiometry (HFA) at different frequencies among study and control groups. There were statistically significant differences in transient evoked otoacoustic emission test (TEOAEs) response among control and study groups. There were no statistically significant differences in absolute and inter-peak latencies of low and high repetition rate ABR among control and studied groups. There were statistically significant differences in speech perception in noise test (SPIN) among the control and study groups.Conclusion: : exposure to recreational noise due to excessive PLDs usage cause elevated EHFA thresholds, decreased TEOAEs amplitudes and decreased SPIN scores despite normal conventional PTA thresholds.

Value of Mastoidectomy in Malignant Otitis External

Objectives: To define the value of mastoidectomy in cases of malignant otitis externa.Study design: Case Series.Patients and Methods: Study of Patients with malignant otitis externa (MOE) treated by mastoidectomy from Jan 2016 to October 2018 and followed up clinically, laboratory and radiologically.Results: Postsurgical siginifacant resolution of symptoms and regression of the disease. Otalgia showed siginificant decrease by assessment with Visual Analogue Score. Also there are improvement of otorrhea, decrease of the reactive indicators as Erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) and radiological improvement of osteomyelitis in Computed Tomography and technetium-99m methylene diphosphonate (MDP) and gallium-67 citrate scintigraphy.Conclusion: In patients with MOE, resistant to medical treatment and have evidence of mastoiditis and Middle ear affection, cortical mastoidectomy may be the treatment for both symptom relieve and regression of the disease

Use of Butterfly Composite Conchal Cartilage Graft with Local Posteriorly Pedicled Flap in Repair of Nasal Septum Perforations

Background : Nasal septal surgery is the most frequent cause of nasal septal perforations (NSP). Sometimes when symptomatic they need repair. There are many methods described for repair with a variable percent of success. The multiplicity of techniques and approaches reflect the difficulty of repair.Objective: To describe a new effective method to repair NSP using a butterfly composite conchal cartilage graft (BCG) with local posteriorly pedicled flap in repair of NSP.Patients and Methods: Eleven patients with NSP after nasal septum surgery were involved in this study. In all of them an ample flap, right sided, pedicled posteriorly on sphenopalatine artery was fashioned. It was rotated to cover the perforation on the right side. A composite skin choncal cartilage graft equal to NSP diameter is dissected. The cartilage graft was fashioned by splitting its edge circumferentially with a depth of 2-3 mm and was positioned with the skin island lying on the left side and the fissured graft edge embracing the perforation cartilaginous border inside its fissure. The graft and the flap were fixed in place with quilting and mattress sutures and silastic sheets on both sides. Mucosal flap was used alone in three cases with small perforations.Results: Complete closure of NSP was achieved in 10 cases (91%). Only one case the graft was rejected with partial closure of the perforation due to bad fixation and secondary infection.Conclusion: BCG was an excellent graft to repair NSP if associated with posteriorly pedicled mucosal flap supplied by the sphenopalatine artery.

Myringotomy with versus without grommet tube insertion in chronic serous otitis media with effusion : Southern Oman experience

Objective: Otitis media with effusion (OME) can lead to hearing loss and delayed speech. Adenoidectomy with myringotomy and ventilation tube insertion are used in the management of OME. This study aimed to compare outcomes of myringotomy with versus without grommet tube insertion in children with OME in Southern Oman.Patients and Methods: A prospective randomized study of 74 patients with serous OME, divided into 2 equal groups, was conducted over 2 years. Group A patients underwent adenoidectomy with myringotomy and grommet tube insertion, while only adenoidectomy and myringotomy were performed on patients in group B. Average age, gender, preoperative hearing loss and postoperative hearing gain, tympanometry 9 months after the operation, and incidence of otorrhoea and myringosclerosis were compared.Results: All the parameters showed a nonsignificant statistical difference. Otorrhoea and myringosclerosis incidence was higher in group A, with a significant statistical difference.Conclusion: Myringotomy alone showed favourable results with fewer complications in treatment of serous OME

Speech Therapy in the Treatment of Globus Pharyngeus: Development of a Mobile Application to Improve Patient Access

Introduction: Globus pharyngeus is a well-known disorder accounting for 3-4% of ENT referrals. Various treatment strategies have been proposed for this condition, including speech and language therapy. The aim of this article is to highlight the evidence-based approach to the development of a mobile application to improve patient access to speech therapy as a treatment for globus pharyngeus.Patients and Methods: A literature search was performed to identify articles exploring the use of speech therapy as a management option for this disorder. A survey of speech and language therapists (SLT) in the UK to determine patient access to this treatment and their views on the development of a mobile application is also described.Results: Four studies were identified, including one randomised controlled study. All articles demonstrate a significant improvement in symptoms following the use of speech therapy. SLTs from 27 departments responded to the survey. The globus pharyngeus patient workload was variable and in excess of 1000 patients per year in some departments. Most respondents supported the development of a mobile application for speech therapy for use by globus pharyngeus patients.Conclusion: Speech therapy is an effective treatment for globus pharyngeus patients. Speech therapy is not available in all NHS Trusts in England and some SLT departments have a high workload. Alternative methods for the delivery of this effective treatment should be considered including the use of mobile applications. It is important to rule out the possibility of upper aerodigestive tract malignancy before referring a patient for speech therapy.

Noise Induced Hearing Loss in Glass Factory Workers in Delta State Nigeria: An 8 year follow up.

Background: Noise induced hearing loss is a well known recognized hazard in factory workers. This is due to the fact that the level of noise in most factories exceed the WHO acceptable limits. This was an 8 year follow up study on glass factory workers to determine the effect of noise on their hearing status over that period.Materials and Methods: Audiometry was carried out on the factory workers using a diagnostic audiometer. This was repeated after 8 years. All the workers had their ears examined prior to Audiometry which was done in a quiet area in the factory clinic. Thereafter data was collected and analysed.Results: The subsequent screening showed that all the workers had increased threshold of hearing and the Prevalence rate of hearing impairment had increased from 9.1% to 40.8%. All workers admitted to intermittent use of personal protective devices (ear muffs/ear plugs) despite being counselled on their use 8 years earlier.Conclusion: Hearing Conservation measures should be instituted in factories. Pre-employment screening should be mandatory as well as annual audiological screening and the labour laws should be strengthened to ensure adequate compliance by industries.

Gradenigo’s Syndrome a Rare Complication of Acute Otitis Media: Case Report and Literature Revue

Introduction: Gradenigo's syndrome is a rare complication of otitis media and mastoiditis involving the apex of the petrous temporal bone. This syndrome is characterized by a clinical triad of deep facial pain, sixth cranial nerve palsy, and acute otitis media (AOM).Case report: We report a case of a six-year-old girl, with no past medical history, who presented with a sudden diplopia following six days of headache and otorrhea. The diagnosis of gradenigo's syndrome was confirmed by clinical examination and imaging. She made a full recovery after 9 weeks of antibiotics.Discussion: Gradenigo's syndrome is a rare but life threatening complication of AOM.The inflammatory process starts in the middle ear, and spreads from the base to the top of the petrous portion of the temporal bone, producing several diagnostic symptoms.Careful clinical history and physical examination, including imaging, are necessary to make the diagnosis.Appropriate management requires antibiotic treatment and possible surgical intervention

FINE NEEDLE ASPIRATION OF METASTATIC MALIGNANT MELANOMA IN PAROTID GLAND: CASE STUDY

Malignant melanoma which is an aggressive tumor with a high metastasis potential, is the second most frequent tumor that metastasize to salivary glands. Fine needle aspiration (FNA) is a simple, rapid and widely accepted method for diagnosis of salivary gland lesions.In this case report, forty-four years old male patient who admitted to hospital due to a painless mass in the parotid gland, is presented. A FNA was performed and following cytological, histological and immunohistochemical examination of the material, the final diagnosis was malignant melanoma. Deatiled patient history revealed a malignant melanoma of head and neck region which was diagnosed two years ago.Although rare, metastasis of malignant melanoma to salivary glands has been reported in literature. Therefore, patients with primary tumor history should be carefully evaluated for metastasis. Besides, a malignant melanoma can be found in parotid gland in patients with an unknown primary locus. A regressed tumor should also be kept in mind.
